	Annual Fee Fee Includes: Plan Discrimination Testing, Oversight of Plan regarding compliance with the IRS and DOL Regulations, Customer Service, Payments on a weekly basis	$475.00 plus Monthly Fee*
	Monthly Fee Fee Includes: Employee reimbursements, Monthly invoicing, Quarterly benefits reports, Account auditing, IRC compliance information, Customer service   Please choose one of the options:  1. Electronic Payments & Collections Through ACH $8.50/Participant    2. Traditional Payments & Collections Via Check $8.50/Participant plus $2.50/Check
